What is Reconstructive Surgery?

Some patients have congenital or acquired benign conditions that cause blockage of the ureter, and swelling of the kidney. When this occurs, patients can have pain in their flank or back, kidney stones, recurrent infections of the kidney, or diminished kidney function. These symptoms can sometimes be more intense after large fluid volume intake. The location of this blockage can occur anywhere in the ureter, the tube that connects the kidney to the bladder. One of the most common locations of the blockage is at the connection between the renal pelvis and the ureter, called the ureteropelvic junction or UPJ, or further distally near the junction of the bladder.

There can be many causes of these blockages including:

  1. external compression from a mass, scar, or crossing blood vessel
  2. internal blockage from scarring (from prior stones, surgeries, infection, radiation), or injury from another surgery.

The workup for the blockage of the kidney usually starts with a CT scan with delayed images of the ureter that evaluate the drainage. This is oftentimes the best test to evaluate the anatomy of the stricture as well as surrounding structures that could be causing the blockage. Sometimes, this may be followed up with a retrograde pyelogram (shooting dye up into the kidney from the bladder) to better define the location and length of the narrowing. Lastly, a nuclear renal scan will be obtained to quantify the degree of obstruction and to assess the function of the affected kidney. Once the narrowing has been fully assessed, the treatment can be determined. Some narrowings can be treated through an endoscopic approach, although most require robotic surgical repair.

Robotic surgery has revolutionized the way we perform reconstructive surgeries for urologic conditions in pediatric and adult patients. Whether the problem is at the level of the kidney, or down at the bladder, the robotic platform allows the surgeon to correct these problems through with excellent success rates. Because the surgery is reconstructive without the need to extract any large pieces of tissue, the robotic surgery can be performed through small incisions, which translates into less pain, better aesthetics, faster recovery, and with minimal side effects and quick recovery for patients. The outcomes are excellent and approach 95% in success rates.
